Formality wins the day every time for me, until one party decides it's appropriate to be informal. IMHO, it's only good manners, but then, I'm a curmudgeonly old git.

I'm not really fussed either way.
However, it seems to be the onboard etiquette for the stewards to use Sir or Madam, so I'm happy to go with that

The days of a patient being called Mr. or Mrs...... are also long gone, unfortunately, but it works both ways.
I have no idea how old you are but when my OH or I attend a hospital appointment and are called, 'dear,' or by our first name, by a person younger than our daughters,( they are in their 20's), it irks me.
When I first started teaching I addressed all members of staff, who were older than me, as Mr. Mrs. or Miss. Ms. did not enter the equation then.
I would be very happy to call you nurse XXXXX if I was called in return by my title and surname, unless I asked to be addressed otherwise.
I am not old but was brought up to respect my elders and I expect the same respect given to me and my OH.
Sorry, a bit off topic, but hopefully relevant to the discussion as how to address others and how, in turn, they would like to be addressed.

I know its slightly off subject but I really don't like it when a cold caller asks to speak to Mrs Janis Rosser - I reply - speaking and then they say "how are you today Janis" - cheeky monkeys - needless to say the phone is put down pretty sharpish! I do like a bit of respect but after a few days of getting to know the steward or the waiters I don't really mind if they call me by my first name although it is generally ma'am - if it's good enough for the Queen it's good enough for me!!

Our wine waiter told us that they are told in their training that they must not call passengers by the Christian names.

I work with babies, so don't tend to call them miss or mister

I'm guessing that you might work with babies that are with you over a long period of time, and, in those circumstances, then of course you get to know the parents really well; we got to know the staff very well when our 2 decided to surprise us over 10 weeks early!

Yes very true! Were your 2 both ok? Thats pretty early! Yes i think it is a very different situation in that case to something like a cruise (see tryig to keep on topic

Oh dear...
The Mr, Mrs, Ms thing seems to grate on the "older" folk who seem to 'need' (dare I say "demand") some sort of respect from those who are their "servants". on a cruise ship (as they see it!)
From my perspective - I am in the mid 30's age range - I see nothing wrong with first names, be that from cabin stewards or waiters on any cruise ship.
We have to move with the times... my parents would, perhaps, disagree with me!
I can't see the problem with addressing each other by 'familiar' names.

I have to agree with you - a female is either Miss or Mrs (unless there is a title involved - and the female in question wishes not to use said title).
Ms on the other is actually short for miserable - or so I am told!

I well remember sharing a table with a retired Army Officer and his "lady"
It soon became apparent that his "lady" insisted that we were expected to be address by his rank

He politely referred to us lesser mortals by using our surnames
Fortunately he was a very pleasant and interesting gentleman
His "lady" however was a right pain in the butt

Having had the misfortune of being a very junior officer on a cruise ship, my worst experience ever
We had to address all passengers thus "Sir" "Madam" - but in the crew areas this was relaxed
Some quite interesting and clever renaming of that certain passenger
